Requirements for Attending...
- Students MUST be Age 21 or younger
- Students MUST Have a valid Learner's Permit or Driver's License at the time of the class
- Students under age 18 must have a completed parental consent form (1 page) AND LVMS Minor Waiver (2 pages).
We need the original forms back from you! You can mail them to us or BRING the originals with you to the event. You cannot attend if we don't have the originals back!
